Dover Park Hospice

Encouraging people to live well because every moment matters

About Dover Park Hospice

Founded in 1992, Dover Park Hospice is Singapore’s first purpose-built hospice, caring for those nearing the end of life’s journey.

As a non-profit organisation, we open our doors to people of every age, race, religion, and income level. Our team of healthcare professionals and volunteers is wholly committed to their calling to enhance the living, comfort the dying, and support the grieving with compassion and dignity.

Our premises have served as a tranquil sanctuary for over 10,000 patients and families over the years, as we meet their unique physical, emotional, psycho-social, and spiritual needs through our brand of holistic palliative care.

In-Patient Care

We provide:

• A multidisciplinary team of nurses, doctors and medical social workers who provide physical, psychosocial and spiritual support.
• On-call service 24 hours a day, seven days a week
• Regular reviews by nurses scheduled according to patients’ needs
• Visits by the doctors when medical attention is needed
• Advance Care Planning discussions
• Training of family members and caregivers to ensure patient’s comfort at home
• Recommend and assist in equipment loans from vendors if needed
• In 2014, we launched Program Dignity under our Home Care service. This patient-centric initiative provides palliative home care for advanced dementia patients to live out their remaining days with dignity.

Homecare Referrals:
• Dover Park Homecare accepts referrals from Tan Tock Seng Hospital within designated areas within the central part of Singapore. All referrals can be made by doctors caring for the patients via Agency for Integrated Care portal. Upon acceptance of referrals, calls will be made to patients and families to explain the service and to arrange for home visits.

Programme Dignity for Advanced Dementia

In 2014, we launched Programme Dignity under our Home Care service. A collaboration between Temasek Cares, Tan Tock Seng Hospital, the Agency for Integrated Care, and Dover Park Hospice, this special patient-centric initiative provides palliative home care for advanced dementia patients to live out their remaining days with dignity.

Recognising that many of them are suffering in silence with inadequate care at home, Dover Park Hospice established a specialised Advanced Dementia Palliative Home Care Team comprising a resident physician, consulting geriatrician/palliative physician, nurse clinician, and a medical social worker.

This team supports persons with dementia and their families with scheduled home visits and further addresses their needs through phone calls and round-the-clock assistance.
